Job Description

Location: Clinton, IN

Setting: Outpatient Clinic

Duration: 13+ weeks with potential for extension to contract-to-hire

Schedule: Monday - Friday, either 8:00 AM - 4:30 PM or 8:30 AM - 5:00 PM

Patient Volume:

• Average Patients per Day: 12
• Average New Evaluations per Day: 2-3

Patient Population: Primarily adult and geriatric; do see high school athletes as well.

Responsibilities:

• Conduct thorough patient assessments to evaluate their physical condition, functional limitations, and needs.
• Develop and implement individualized treatment plans with achievable functional goals utilizing therapeutic exercise and modalities as necessary.
• Review available patient information and communicate the assessment findings and treatment process effectively.
• Revise treatment plans as needed based on patient progress and response.
• Establish and instruct patients and/or their families/caregivers in exercises and therapeutic procedures for continued care post-discharge.
• Delegate tasks to and supervise Physical Therapist Assistants (PTAs) and rehabilitation technicians.
• Complete all required documentation accurately and in a timely manner according to company/department policies, procedures, and relevant federal and state regulations.

Required Qualifications:

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy.
• Current and valid license to practice as a Physical Therapist in the State of Indiana.
• Current CPR/AED/BLS certification or the ability to obtain prior to the start of employment and maintain valid certification(s).
• Ability to stand, walk, and sit for extended periods throughout the assigned shift.
• Strong computer skills, including proficiency with databases and Microsoft Office Suite (Outlook, Word, PowerPoint, Excel).
• Ability to maintain strict patient confidentiality.
